Human Resources is available to all employees for any questions regarding their employment. This page has been created to highlight important information related to employee compliance with

Anthony Medical & Chiropractic.

Human Resources performs annual employee file maintenance. To assure we have the most current information in your file, please complete the following documents and return them to HR if:

a) you are completing your required annual file maintenance 

b) you have changes that need to be updated in your employee file


If you are a new employee or a newly-hired employee awaiting your start date, please complete and return each of the documents below by email.


You will sign each document requiring signature on your first day.

Receive your income directly into your bank account.  

W-4 Form

Enter your information for your desired tax withholdings

I-9 Form

Fill out this form to verify your residency and authorization to work in the United States.

Employee Information Form

Update your information on file in order to receive all the information you need.
