Human Resources is available to all employees for any questions regarding their employment. This page has been created to highlight important information related to employee compliance with Anthony Medical & Chiropractic.

Human Resources performs annual employee file maintenance. To assure we have the most current information in your file, please complete the following documents and return them to HR if:

a) you are completing your required annual file maintenance 

b) you have changes that need to be updated in your employee file

OR

If you are a new employee or a newly-hired employee awaiting your start date, please complete and return each of the documents below by email.

 

You will sign each document requiring signature on your first day.
